CCS

Risk Management

CCS had decades of proprietary diabetes-care data but needed a practical way to turn it into earlier, more personalized interventions that would improve adherence and reduce the cost of chronic-care management.

50% Improved adherence in targeted, high-risk patient cohorts
85% Accuracy predicting patient behavior almost 3 months in advance
up to US$2,200 per patient per year

Reported impact

Business outcome

CCS moved from retrospective reporting to earlier intervention, improving patient adherence, enabling more informed care-management decisions, and lowering avoidable costs for high-risk diabetes populations.

Solution

Accenture worked with CCS to build a predictive care-management stack centered on two linked assets: the Customer Analytics Record (CAR) and PropheSee, an AI-powered predictive model. The team first analyzed more than two decades of CCS internal data and combined it with social determinants of health information to create a longitudinal patient view in CAR. That unified data foundation was integrated with CCS's existing Azure Data Lake environment so predictive models could be trained and operationalized without rebuilding the broader data estate. Multiple models were evaluated before the team selected a winning model that predicts patient behavior almost three months in advance. CAR data then feeds intervention programs and outreach messaging so CCS can target high-risk patients earlier, tailor engagement, and support treatment adherence with a more proactive chronic-care operating model.

Data flow

CCS internal records and social determinants of health data are combined into CAR and connected to Azure Data Lake. Predictive models in PropheSee score patient behavior and adherence risk, and those outputs trigger intervention programs and messages that help care teams reach high-risk patients earlier.

    • social determinants of health information Augments CCS internal records with external context used to improve prediction quality.
  2. Compute
    • PropheSee Runs predictive models that estimate patient behavior and adherence risk weeks in advance.
  3. Storage
    • Customer Analytics Record (CAR) Creates a longitudinal patient record that unifies CCS data for predictive care and outreach decisions.
    • Azure Data Lake Hosts the broader data environment into which the predictive program integrates.
  4. Serving
    • intervention messaging programs Turn predictive outputs into patient outreach that encourages adherence and follow-through.

Architecture clues

  • Multiple predictive models were evaluated before production selection.
  • The model uses a longitudinal patient record built from internal and external data.
  • The predictive program integrates into CCS's existing Azure Data Lake environment.

Scoring methodology

How scores are calculated

Customer Reference Value measures usefulness as a customer reference. Evidence Confidence measures the quality and traceability of available evidence, not independent verification.

Customer reference value100/100
  • Problem clarity20/20
Measures whether the customer's operational problem is specific enough to compare.
  • Implementation depth25/25
Rewards named components, data flow, integrations, and architecture detail.
  • Outcome specificity20/20
Measures whether the reported benefit is concrete rather than a generic claim.
  • Quantified impact25/25
    2 metrics describe measurable impact.
  • Deployment scale10/10
Rewards evidence of a live or scaled deployment and meaningful integration scope.
Evidence confidence93%
  • Source authority20/25
The main evidence is an official vendor-published customer story.
  • Claim traceability23/25
11 of 12 key fields link to a named source.
  • Direct evidence20/20
    4 direct source quotes retained.
  • Date provenance10/10
The publication date comes from the source page or publisher.
  • Source transparency10/10
3 source references document publisher, URL, authority, retrieval, and supported fields.
  • Implementation specificity10/10
Rewards source-backed components, data flow, quantified impact, and a specific outcome.
